Denied, delayed, underpaid, or unresolved government-funded claims can represent significant revenue your facility may have already earned. RCCS helps providers review the problem, prepare the appeal, organize supporting documentation, communicate with the payer, and pursue eligible reimbursement.
Government-funded billing does not always end when the original claim is submitted. Claims may be denied, partially paid, delayed, rejected, or left unresolved because of documentation questions, billing issues, payer processing requirements, authorization concerns, or other administrative reasons.
RCCS helps facility owners and providers organize the case and pursue eligible reimbursement through the appropriate administrative claim and appeal process.
Our compensation model is intentionally structured so that the majority of our fee is tied to the money actually recovered.
The Administrative Deposit applies separately to each facility enrolled in RCCS claims recovery services.
A facility is identified based on the facility number issued by the California Department of Social Services or the facility's applicable NPI number.
An owner or organization operating multiple separately identified facilities must establish a separate Administrative Deposit for each participating facility.
The applicable Administrative Deposit is credited dollar-for-dollar against RCCS's recovery fee for that facility.
The Administrative Deposit allows RCCS to begin reviewing the claim history, organizing the file, researching payment issues, preparing appeal materials, and committing professional resources to the recovery effort.
When RCCS successfully recovers money, 100% of the Administrative Deposit is credited toward the applicable recovery fee.
If no recovery is achieved within 12 months after the Complete File Date, the Administrative Deposit is refundable when the provider has fully satisfied the cooperation, documentation, authorization, exclusivity, and other requirements of the signed service agreement.
The Administrative Deposit is not refundable when noncollection is caused or materially contributed to by client nonresponse, false or incomplete information, withheld documentation, interference with the appeal, missed client obligations, or violation of RCCS's exclusive engagement requirement.
RCCS's Billing Claims and Appeals Department includes a Senior Consultant with 42 years of experience in billing and claims appeals.
That experience helps RCCS evaluate claim history, identify potential administrative issues, organize supporting information, and develop a structured approach to recovery.
Every claim remains subject to payer rules, available documentation, filing limitations, appeal rights, and the specific circumstances of the provider's case.
In prior claims-recovery work, we have successfully collected eligible claims with a lookback period of up to 18 months at 100% of the collectible claim amount.
That does not mean every payer or every claim allows an 18-month lookback. Insurance companies and government-funded plans can have different timely filing, reconsideration, reopening, and retroactive payment rules.
A claim should not automatically be written off simply because several months have passed. RCCS can review whether an available recovery or appeal path may still exist.
Certain payers may reduce the collectible reimbursement percentage as the claim becomes older, even when retroactive payment remains available.
Actual eligibility, lookback period, allowable reimbursement, timely filing rules, and appeal rights depend on the specific payer and claim.
A payer may allow an older claim to be recovered but reimburse only a percentage of the otherwise collectible amount. For example, a payer's rules might provide that a claim submitted or corrected seven to nine months after the applicable service date is eligible for only 75% of the claim amount. These percentages vary by payer and are not controlled by RCCS.
In this example, RCCS's total 15% recovery fee is $7,500. Because the facility already paid the $1,000 Administrative Deposit, only the remaining $6,500 is due after successful recovery.
Applies to eligible engagements with contracts fully signed no later than December 31, 2026 at 11:59 PM Pacific Time.
The $1,000 Administrative Deposit is charged per participating facility and is credited toward the applicable recovery fee.
New engagements beginning January 1, 2027 will be subject to the increased Administrative Deposit and recovery-fee structure.
The $1,500 Administrative Deposit will likewise apply per participating facility, subject to the applicable service agreement.
RCCS's recovery fee applies to the defined amount successfully recovered on a Covered Claim under the signed service agreement.
The applicable RCCS recovery fee must be paid within 14 calendar days from the date the insurance provider or payer issues the check, EFT, payment, credit, offset, or other defined recovery.
The provider must supply applicable remittance, payment, check, ERA/EOB, portal, or similar documentation needed to verify the recovery amount and calculate the RCCS fee.
Once RCCS accepts a Covered Claim, RCCS must be the exclusive claims appeal and recovery provider for that claim during the applicable engagement period.
This helps prevent competing submissions, conflicting explanations, duplicated appeal activity, or multiple companies attempting to pursue the same claim at the same time.
When RCCS has materially worked a Covered Claim and the claim later results in Recovery during the applicable engagement or recovery-tail period, RCCS's recovery-fee rights remain subject to the signed service agreement even if another party later becomes involved.
Every claim is subject to RCCS review and acceptance. RCCS does not guarantee payment, approval, recovery amount, processing time, or appeal outcome.
The 2026 $1,000 Administrative Deposit applies per participating facility, determined by the applicable CDSS facility number and/or NPI used to identify that facility.
The 15% introductory recovery fee applies only to eligible engagements with contracts fully executed no later than December 31, 2026 at 11:59 PM Pacific Time. New engagements beginning January 1, 2027 are subject to a $1,500 per-facility Administrative Deposit and 20% recovery fee.
The Administrative Deposit is credited toward the applicable recovery fee. Administrative Deposit refund protection is subject to the Complete File Date, provider cooperation requirements, exclusivity requirements, documentation requirements, and all other conditions of the signed RCCS service agreement.
Client-caused delays may pause the applicable 12-month Administrative Deposit Protection period. The Administrative Deposit may become nonrefundable when the provider fails to reasonably cooperate, repeatedly fails to respond, withholds required records, provides materially inaccurate or false information, interferes with the appeal, misses required client actions, or engages another claims-appeal provider in violation of the exclusivity requirement.
Recovery-fee invoices are due within 14 calendar days from the payer's applicable payment or recovery issuance date as defined in the signed service agreement.
Prior successful claim recoveries, including recoveries involving extended lookback periods, do not guarantee that another payer will permit the same lookback period or reimbursement percentage. Payer rules may impose filing limits, appeal limits, reduced retroactive reimbursement, tiered collectible percentages, or other restrictions.
RCCS provides administrative claims recovery and appeal support. Formal legal proceedings, litigation, government investigations, regulatory defense, or other matters outside the administrative service scope may require separate qualified legal or professional representation.
The signed RCCS Government-Funded Claims Recovery Agreement controls final definitions, covered claims, rates, Administrative Deposit protection, exclusivity, recovery-tail provisions, provider responsibilities, payment obligations, refund conditions, and all other engagement terms.
